Digital Health for Interventions, Health Equity, and Member-Focused Engagement
With AHIP 2024 in our rearview, we have had time to reflect on our experience at the conference and the many interactions with industry professionals we met. This year’s convention was vibrant—the energy could be felt from start to finish and the event was packed with key players from across the healthcare industry. AHIP always highlights the future of healthcare, and this year was no different. We saw three main topics recurring throughout the event: digital health for interventions, health equity, and member-focused engagement.
Digital Health for Interventions
At the heart of the AHIP conference is continuously exploring ways to improve health. This year, digital health took center stage, showcasing how technology can be harnessed to gain insights and lead to interventions for better outcomes. The World Health Organization (WHO) states that “digital health can help make health systems more efficient and sustainable.” The Connect America CareSage analytics and reporting platform is a perfect example of digital health not just providing data and communications but taking that data a step further and providing actionable information and insights. Our insights, such as mobility details, risk assessment, and other customizable data, take the best of digital health and put it to work, facilitating better outcomes while reducing costs. In a study of more than 370 individuals utilizing PERS coupled with the CareSage platform, our partners saw a 68% reduction in 90-day hospital readmissions, a 53% reduction in 180-day hospital readmissions, and a 31% decrease in hospitalization costs.
Health Equity
The COVID-19 pandemic has starkly exposed the inequities in our healthcare system. In response, major players in the industry are asking how we can bridge these gaps and create equity where it is lacking. While numerous solutions have emerged to tackle health equity issues, Connect America remains unwaveringly committed to effectively addressing these challenges. Our unique window into the home, provided by PERS and RPM, allows our partners to collect data to determine where inequities exist. Once identified, we can help to address these needs with our engagement services to facilitate better outcomes and reduce costs. Rosemary Kennedy, our Chief Informatics Officer, stated in a Healthcare IT article that Connect America can help facilitate “the collection of data related to social determinants of health including food, medications, or transportation, to help more effectively manage their care.”
Member-Focused Engagement
Engagement was key at this year’s AHIP conference. Plans consistently seek ways to engage more with participants, enhance satisfaction, and retain existing members.
